SOCIAL PRESCRIBING LINK WORKER

Full time Post

Based in Lowestoft

Salary – £25,991 p.a.

We are looking to recruit an exceptional person to join our Social Prescribing Team.

Working within the primary care network multi-disciplinary healthcare teams in Lowestoft the successful candidate will provide 1:1 personalised support to people referred to them by team members and local agencies.

The successful candidate will have:-

 

  • Excellent listening skills;

 

  • Be committed to supporting local communities to care for each other;

 

  • Experience of working with people facing complex social and emotional challenges

 

  • Great interpersonal skills in supporting people, community groups and local organisations.

 

  • be able to demonstrate that they are flexible, confident, self-motivated team players.

 

The is full time however we are happy to receive applications from people wishing to job share/work part-time
